Overview
Community Care Solutions is a recruitment/placement service
matching personal care workers with clients who need qualified
and reliable persons to provide care for their loved ones.
We maintain a registry of skilled
individuals trained to provide personal care, child care and
domestic care on short- or long-term assignments, either on a
part-time basis, or in-home on a full-time basis.
Using a consultative approach with
clients and families, we match your needs to qualified care
workers with the specific professional qualifications and
skills you require.
Our goal is to match you with a trained
professional for either live-in or part time caregiving. We
want to ensure that our clients can continue to enjoy
independence through our services.
Who
are we?
Community Care Solutions is a division of
Creative Career Systems. Since 1989 Creative Career Systems
has been setting educational standards and training Certified
Caregivers/Personal Support Workers and Professional Childcare Workers.
Our graduates have both classroom and on-the-job skills
training.
Community Care Solutions is a placement
agency. Our registry of personnel includes graduates of the
Personal Support Worker Programs offered through the CCG
school membership in
Canada
, the
Philippines, India
and
China
, along with other trained health care workers including RPNs
and RNs.

Our
Caregivers
Community Care Solutions maintains a
registry of qualified caregivers with a variety of skills and
qualifications to meet any client's specific needs.
Our standard of caregiving is exemplified
in the Certified Caregiver/Personal Support Worker. Certified
C/PSWs must complete the six-month training program and
on-the-job placement experience. Training includes nursing
care, home management and community social work. Our
international candidates have met the requirements of the
Canadian Embassy in the
Philippines
. They have earned their Canadian diploma as Certified
Caregiver/Personal Support Workers and have completed the Canadian
National Association of Certified Personal Support Workers
examination. International candidates have a Grade 12 equivalent
and have completed health and police checks.
Our recruitment process involves
pre-screening all candidates for listing on the registry,
followed by interviews based on the specific requirements of a
client, including the identification of desired personality
traits. All international students require six months of
training (Canadian students require 24 weeks). All must write
a certification exam and submit police background and health
checks to the National Association of Certified Personal Support Workers
to ensure quality control.

Each caregiver
must go through a supervised 120-hour training placement to
monitor and 160 hours of institutional and community
Internship, to ensure the quality of service.
The
process
The first step in the process for individuals or families
seeking a personal care worker is an in-home assessment by our
qualified multi-disciplinary team, including nursing, social
work, occupational therapy, and physical therapy, as required.

Once a client's requirements are defined,
CCS will select a candidate who meets all of those
requirements. There will be an eight-week probationary period
to ensure that there is a match between the client and
caregiver. In the unlikely event of a mismatch, a new
candidate will be offered. (Note: personal information on
clients and candidates will only be forwarded with the
client's or the candidate's permission and payment of
applicable fees).
The
assessment process
The assessment is an in-home evaluation
done by a multi-disciplinary team (as required) to determine a
client's specific care needs. It will identify high-risk
clients with regard to safety for both the client and the
caregiver. It will also establish a baseline in order to
ensure consistency in care and to allow caregivers to monitor
and identify changes in the client's condition.
The assessment is done in-home for the
comfort and privacy of the client, and to allow the assessment
team to evaluate the client in familiar surroundings. It will
include an evaluation of communication, encompassing verbal
and non-verbal communication and sensory-cognitive deficits
(vision or hearing impaired).
The assessment consists of a short
interview, a questionnaire and observations by the assessment
team.
Components
of the assessment
The assessment will address the
following:

Overall physical health — body systems, sleep
patterns, nutritional levels, past medical history, family
history.
·
Activities of Daily Living (ADLs) — dressing,
grooming feeding, toileting, financial management.
·
Instrumental Activities of Daily Living (IADLs)
— shopping, meal preparation, medicine management,
transportation.
·
Advanced Activities of Daily Living (AADLs) —
social and recreational activities (crafts, gardening,
hobbies)
·
Safety Assessment of Home — lighting,
flooring, assistive devices in place or required, stairs,
security.
·
Social Support — family, number of caregivers,
potential abuse.
Employer
services
Community Care Solutions works with employers to develop the
position’s requirements, and to define the suitable
educational, experience and personality qualifications a
candidate will require for a successful match. By
understanding fully your needs, we can establish the best
"fit" between the client and candidate.
CCS
Recruiting
Community Care Solutions maintains a registry of qualified
personnel. Once the requirements of a client are established,
we conduct an inspection of the client’s home. We also
contact our North American candidate referral network and
initiate any necessary advertising or promotional activity
through the internet and in appropriate publications.
Through our detailed candidate search we
provide an accurate matching of individual skills and
abilities to the client's requirements. We take into
consideration personality trait requirements and will
administer or coordinate any candidate testing that may be
required.
Selection
Process Assistance
Community Care Solutions works with all
parties involved to ensure a successful recruitment and
placement. We will seek additional information as required;
facilitate interviews and provide interview feedback, and
conduct a thorough position-based reference evaluation,
including previous employer comments, employment date,
education verification and past salary history.
As well we will assist with
employer-candidate in contract and salary negotiations.
Communication
between Clients and Candidates
All communication between Clients and
Candidates (email, written, verbal) is made through CCS until
each party agrees to direct contact. At this time CCS requests
an agreement between client and caregiver.
